WAIVER AND MEDICAL TREATMENT CONSENT: Recognizing the possibility of personal injury associated with participation in the above-named class provided by Spokane Art School, I hereby assume that risk, and waive and release Spokane Art School, and any of Spokane Art School’s instructors, employees, officers, or volunteers, from liability for any personal injury that I may suffer or experience arising out of my participation in the class. This waiver and release apply to any Spokane Art School activity, including but not limited to classes held at Spokane Art School and classes held at any off-site location designated by Spokane Art School. I hereby give my consent to receive any medical assistance and/or treatment that I may require during my participation in the class.

PHOTO/MEDIA RELEASE: The Spokane Art School has my permission to use my likeness, and/or that of my child, publicly to promote the Spokane Art School. I understand that the images may be used in print publications, promotional materials, online publications, presentations, websites, and social media. I understand that no royalty fee or other compensation shall become payable to me by reason of such use.
