Fall Art School Class Proposals
Please fill out a seperate form for each potential class, unless it would be multiple occurances of the same curriculum.
Instructors are required by the City of Spokane to have a valid business license. We will also conduct a WSP background check and require you to sign and follow our Code of Conduct. If you have not taught with us previously, we will need a W-9.
